HCF owns and operates HCF Eyecare Centres (HCF Eyecare).
HCF Eyecare requires you to complete the Pre-Examination Questionnaire to complete about yourself, or on behalf of a patient on whose behalf you have the authority to complete the questionnaire. By completing the Pre-Examination Questionnaire and submitting to HCF Eyecare you acknowledge that you have read, understood and agree to, where applicable, the information set out below and your responses to the Pre-Examination Questionnaire are true and complete.
Any references to personal information includes sensitive information such as health information or a health record.
Patient Consent
You authorise HCF Eyecare to collect and access your personal information, or have the authority or consent of the patient where necessary, to authorise HCF Eyecare to collect and access their personal information and for you to receive their information from HCF Eyecare, for the purposes of providing eye care and related health care services.

- HCF Eyecare collects your personal information you submit in the Pre-Examination Questionnaire to assess your eye care needs and related health requirements.
- Failure to supply any of the information requested in the Pre-Examination Questionnaire may mean that you will not be able to receive some or all of the eye care and related health care services.
